Behavior Policy

Brookstone Summer operates under the philosophy that involves well-understood standards of conduct. The Directors appreciate working with students on a basis that promotes a cooperative relationship to the extent that it is practicable. It is essential that each student accept total responsibility for his/her conduct at all times. Reminder: All attendees must be potty trained.

Cancellation and Transfer Policy

There will be a $25 fee for all cancellations made less than ten calendar days prior to the start of the camp. However, exchanges for another camp are permissible if space is available.

In order to transfer to another camp you must go online and register for the camp you wish to transfer to. A refund will be issued to your credit card for the camp that you transferred from.

Brookstone Summer reserves the right to cancel any camp that does not meet the minimum attendance requirement. Please check each registration page to see if space is available.

Emergency Medical Authorization

I hereby authorize Brookstone Summer to secure medical attention and care for my child as deemed necessary. I shall assume responsibility for payment of services. I agree to keep the camp office informed of changes in telephone numbers, etc. where I can be reached.

Brookstone Summer agrees to keep me informed of any incidents requiring professional medical care involving my child.

Waiver of Liability and Participation Information

This is a legally binding Consent Form and release of liability made voluntarily by me on my own behalf, and on the behalf of my heirs, executors, administrators, legal representatives, and assigns to the Board of Trustees of Brookstone Schooland its institution, Brookstone School and Brookstone Summer (all of the prior listed releasees are hereinafter referred to as "Brookstone School").

I acknowledges that participation in Brookstone Summer camp activities involves inherent risk of physical injury and assumes all such risks. I understand that some of the activities may involve unleveled ground, heat, humidity, rigorous physical activity and contact with allergens, plants and animals. As the School has made available equipment, facilities, grounds or personnel for such programs or activities, the undersigned understands that participation in Brookstone Summer activities does hereby release and forever discharge Brookstone School and the Board of Trustees, its members individually, and its officers, agents and employees of any and from all claims, demands, rights and causes of action of whatever kind of nature, arising from and by reason of any and all unknown, foreseen and unforeseen bodily and personal injuries, damage to property, and the consequence thereof, resulting in participation in Brookstone Summer activities.

Transporting of students to various sites may require transportation in Brookstone School vehicles. Transportation may include vehicles such as buses, vans and golf carts. I understand that these vehicles may not have seat belts. I agree that, even if the vehicles have seat belts, Brookstone School is not responsible for the proper use of such seat belts. I understand that transportation involves an inherent risk of physical injury.

In an emergency, I acknowledge that I am solely responsible for all medical and other costs arising out of bodily injury or any loss sustained through participation in Brookstone Summer activities. I authorize program staff to secure any licensed hospital, physician, ambulance, and/or medical personnel for treatment deemed necessary for the participant's immediate care.

By accepting this agreement, I agree that this release includes physical injury, death, property damage, or emotional harm caused by negligence of the employees, agents, officials and trustees of Brookstone School when the law allows for a defense of immunity under the Recreational Property Act, the State Tort Claims Act or any applicable statute or law. I agree that this release does not include willful misconduct by Brookstone School and its agents, employees, officials and trustees; however Brookstone School is not liable for the criminal acts of third parties. Should Brookstone School or anyone acting on its behalf be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold Brookstone School, and anyone acting on its behalf, harmless for such fee and costs.

I have read the above carefully and I understand that this agreement covers all Brookstone Summer sessions attended by the participants in this current year.

Financial Policies / Payment

The preferred and fastest method of camp registration and payment is on-line through the Brookstone web site by using a Visa or MasterCard. Payment is due in full prior to the start of camp. No credit will be extended. To pay by check please pick up a registration form from the school office and submit the completed form with payment. Please make checks payable to Brookstone School.

Prorated fees or refunds: Under normal circumstances Brookstone Summer will not prorate or refund fees once camp session has begun.

Media Release Agreement

I do hereby authorize Brookstone Summer to copyright and or use and /or publish for any article, advertisement, electronic media, television commercial, trade or any lawful purpose whatsoever, photographic portraits or pictures of my child(ren) (or talent by them), for which they may be included in whole or in part.
